2996661623-2 Apply for this job now Job Description Hartford Healthcare seeks a Campus Care Hybrid Psychiatric APP to deliver compassionate mental health services across campus and virtual settings. In this role, you will conduct psychiatric evaluations, manage medication, and develop treatment plans for diverse patient populations. Collaborate closely with psychiatrists, therapists, and primary care teams to ensure coordinated, evidence-based care. You will participate in case conferences, maintain accurate clinical documentation, and support crisis intervention when needed. Responsibilities Provide psychiatric evaluations for campus-based and virtual patients Develop and manage individualized treatment and medication plans Collaborate with psychiatrists, therapists, and primary care providers Document clinical encounters in compliance with regulatory standards Participate in interdisciplinary case conferences and care coordination Support crisis intervention and risk assessments as needed Educate patients and families on treatment options and wellness Utilize evidence-based practices and clinical guidelines Contribute to quality improvement and patient safety initiatives Engage in ongoing professional development and training Required Skills Psychiatric assessment Medication management Treatment planning Crisis intervention Psychopharmacology Electronic Medical Record (EMR) documentation Interdisciplinary collaboration Risk assessment and safety planning Patient and family education Knowledge of evidence-based mental health practices
