Job Description

SCOPE OF WORK
The Out Patient Counselor provides counseling to patients in an out-patient facility, which are chemically dependent and develops a treatment program on individuals needs. Will assist patients in developing an understanding of chemical dependency and facilitates effective methods to maintain a Drug-Free lifestyle.
KEY DUTIES & RESPONSIBILITIES
Conduct substance use disorder screenings, assessments, and evaluations in accordance with program policies and procedures.

Complete comprehensive biopsychosocial assessments, substance use histories, and intake interviews.

Utilize DSM-5 and ASAM Criteria to assist in determining appropriate levels of care and treatment recommendations.

Coordinate and facilitate the admission process for eligible clients.

Develop preliminary treatment recommendations based on assessment findings.

Coordinate referrals to inpatient, outpatient, aftercare, mental health, medical, housing, employment, educational, legal, social service, and community resources.

Provide crisis intervention and immediate support for clients presenting with urgent behavioral health or substance use concerns.

Maintain accurate, complete, and timely client records in accordance with program requirements and professional standards.

Participate in multidisciplinary team meetings, staffing, case reviews, and treatment planning discussions.

Bachelor's DegreeValid Drivers LicenseSHIFT:
Monday through Friday, 8:00 a.m. to 4:30 p.m.
