QualificationsBachelor's Degree in a related field.
Job-related experience in family outreach, community resource coordination, or education support preferred.
General ResponsibilitiesAssess home, community, and school conditions impacting a student's social and academic achievement, including home visits if appropriate.
Develop referral pathways for community resources and related services, and assist families in the referral process.
Serve as a liaison between the school, community partners, and foster care systems.
Assist school administrators and the student support/wellness team(s) with school-wide social-emotional learning support and prevention-focused programming.
Services to StudentsProvide attendance support and intervention strategies to increase academic and social success.
Facilitate "Lunch Bunch" and other non-clinical student support groups.
Support students in developing and utilizing appropriate social interaction skills through prevention-focused activities.
Conduct suicide risk assessments and safety planning (with support).Coordinate telehealth support, communication, and access for students.
Services to Parents/FamiliesConduct dedicated family outreach and engagement activities.
Lead family meetings focused on resource identification and collaborative problem-solving.
Facilitate caregiver support groups.
Work with parents/guardians to facilitate their support in their children's academic and social well-being, helping to alleviate family stress.
Assist parents/guardians in accessing and utilizing school and community resources, programs, and basic needs assistance.
Services to School PersonnelProvide staff with essential information to better understand societal, cultural, economic, and familial factors affecting a student's performance and behavior.
Collaborate with teachers and student support staff to strategically align community and family resources.
Provide direct support to staff regarding attendance intervention plans and family engagement strategies.
School-Community LiaisonObtain and coordinate community resources, donations, clothing closets, school supplies, and holiday assistance to meet students' and families' needs.
Organize community partner collaborations and resource fairs.
Advocate for improved community and school services to meet the diverse needs of students and families.
Services to the DistrictCoordinate alternative efforts and support plans for students with attendance concerns or truancy issues.
Identify and report child abuse and neglect.
Provide case management and coordination for families requiring multiple community resources and partnerships.

Building PrincipalJob OverviewThe Family Support and Community Resource Coordinator works with students, families, and school personnel from all grade levels within the district (K-12). As a member of the student support and wellness team, this individual serves as a vital link between home, school, and community to promote and support students' academic and social success. The primary focus of this role is on prevention, connection, coordination, and family support. This position provides essential family engagement, attendance support, resource coordination, community partnership development, telehealth coordination and support, and prevention-focused programming.QualificationsBachelor's Degree in a related field.
