Crisis Counselor / Peer 3p-11p Full Time Bachelors = Mobile Crisis Counselor Experience but no degree = Mobile Crisis Peer Assist Crisis Clinicians with crisis evaluations, including identifying client support systems, protective factors, and risk factors. Conduct follow-up outreach and stabilization services for individuals following a mental health crisis. Provide ongoing support until clients are stabilized, connected to appropriate community behavioral health services, or have completed the follow-up period. Utilize personal lived experience or experience supporting someone with mental illness to engage clients and foster meaningful connections. Support clients in developing and implementing individualized recovery and wellness plans. Promote self-advocacy, self-care, resilience, and overall wellness among clients. Deliver client-centered support services in the setting most comfortable and accessible for the client and their family. Collaborate with hospital staff, behavioral health providers, and community partners to ensure continuity of care and successful service transitions. Connect clients and families with community resources and support services that enhance recovery and stability. Build trusting relationships that encourage engagement in treatment and long-term recovery efforts. Document client contacts, interventions, and progress in accordance with organizational and regulatory requirements. Participate as a member of a multidisciplinary crisis response team to provide comprehensive support and care coordination.
Requirements:
