• Job Summary
• The Case Manager position involves providing comprehensive case management services to individuals in need, with a focus on workforce development, crisis management, and behavioral health support. The role requires working closely with clients to address their needs and connect them with appropriate resources.
• Duties
• Supervise and coordinate services for clients, ensuring their well-being and progress
• Utilize motivational interviewing techniques to engage clients and support positive outcomes
• run groups in line with substance use and mental health recovery
• Conduct assessments and develop individualized care plans based on client needs
• Collaborate with healthcare providers to ensure access to necessary medical services
• Offer addiction counseling and support for individuals struggling with substance abuse
• be able to document appropriately and efficientily in EMR system
• Can run mental health and substance use groups, and be able to handle a case load of up to 12 clients.
• Experience
• Previous experience in case management or a related field is preferred
• working with women in the mental health/substance abuse field
• Strong communication skills and the ability to work effectively with diverse populations
• required
• regsitered with CCAPP/ CAADE/or CADPT.
• Preferred to be certified
