- Job Summary
- The Case Manager position involves providing comprehensive case management services to individuals in need, with a focus on workforce development, crisis management, and behavioral health support. The role requires working closely with clients to address their needs and connect them with appropriate resources.
- Duties
- Supervise and coordinate services for clients, ensuring their well-being and progress
- Utilize motivational interviewing techniques to engage clients and support positive outcomes
- run groups in line with substance use and mental health recovery
- Conduct assessments and develop individualized care plans based on client needs
- Collaborate with healthcare providers to ensure access to necessary medical services
- Offer addiction counseling and support for individuals struggling with substance abuse
- be able to document appropriately and efficientily in EMR system
- Can run mental health and substance use groups, and be able to handle a case load of up to 12 clients.
- Experience
- Previous experience in case management or a related field is preferred
- working with women in the mental health/substance abuse field
- Strong communication skills and the ability to work effectively with diverse populations
- required
- regsitered with CCAPP/ CAADE/or CADPT.
- Preferred to be certified
