This role focuses on assertive outreach, client engagement, and barrier reduction. The goal is to help clients navigate community resources, decrease crisis episodes, and support long-term recovery and community stability.

Key Responsibilities Outreach and Engagement:

Establish trusting relationships with clients through face-to-face, telephone, or community-based outreach.

Case Management:

Deliver comprehensive case management services, assisting clients in accessing ongoing medical, psychiatric, and social services.

Resource Navigation:

Help individuals navigate local Illinois social safety nets, housing, food assistance, and behavioral health treatment programs.

Crisis Support:

Participate in team-based or community responses to individuals experiencing acute mental health stressors, assisting with de-escalation and supportive interventions.

Care Coordination:

Collaborate closely with a multidisciplinary team including counselors, nurses, psychiatric providers, and external partners.

Documentation:

Ensure accurate and timely completion of clinical and administrative documentation in the Electronic Health Record (EHR) system within agency timeframes.

Advocacy:

Serve as an advocate and mentor for clients, empowering them to actively participate in their own treatment and recovery plans.

Qualifications & Requirements Education:

Bachelor's degree in Social Work, Psychology, Counseling, or a closely related human services field preferred; minimum qualifications as a Mental Health Professional (MHP) per Illinois Department of Human Services Rule 132.

Experience:

1 to 2 years of experience working in behavioral health, social services, or crisis intervention preferred. Lived experience in mental health or substance use recovery is a welcomed asset for certain peer-integrated tracks.

Certifications:

Valid Illinois driver's license, reliable personal transportation, and proof of auto insurance required for community visits. Ability or willingness to obtain a Certified Recovery Support Specialist (CRSS) credential if designated.

Skills:

Strong interpersonal and active listening skills; working knowledge of local Illinois community resources; basic proficiency with electronic health records and Microsoft Office.

Pay:

From $30.00 per hour Expected hours: No more than 40.0 per week
