Job Description

Overview Join our dedicated team as an Intensive In-Home Team Lead, where you will play a vital role in delivering comprehensive, family-centered care to children and individuals with developmental disabilities, behavioral health needs, and mental health challenges. This dynamic leadership position offers the opportunity to guide a compassionate team in implementing evidence-based interventions within clients' homes and communities. Your expertise will foster positive change, promote independence, and support families through personalized treatment plans. Duties Lead and supervise a team of behavioral health specialists providing intensive in-home services, ensuring high-quality care aligned with individualized treatment plans. Coordinate and deliver evidence-based therapies such as behavioral therapy, cognitive behavioral therapy, applied behavior analysis, and trauma-informed care tailored to each client's needs. Conduct comprehensive patient assessments, diagnostic evaluations, and progress monitoring while maintaining detailed medical documentation compliant with HIPAA regulations. Provide crisis intervention and conflict management to de-escalate challenging situations effectively while ensuring safety for clients and staff. Collaborate closely with families, schools, healthcare providers, and community agencies to develop and implement effective case management strategies. Oversee data collection efforts related to client progress and treatment outcomes to inform ongoing care adjustments. Ensure all team activities adhere to medical terminology standards, behavioral health protocols, and legal requirements related to patient care and confidentiality. Skills Proven experience working with individuals with mental health diagnosis, behavioral health needs, substance abuse, trauma, developmental disabilities, autism spectrum disorder, or other special needs populations. Strong knowledge of motivational interviewing techniques and crisis management strategies. Expertise in behavioral health modalities such as applied behavior analysis (ABA), psychotherapy, social work practices, and child & family counseling. Skilled in conflict management, behavior management, and home & community care coordination. Familiarity with medical records systems, medical terminology, HIPAA compliance, and documentation standards. Excellent case management skills with the ability to conduct patient assessments and diagnostic evaluations accurately. Effective leadership qualities and motivating staff toward shared goals. Pay:
$60,000.00 - $70,000.00 per year
