Assist with developing individualized person-centered plans in collaboration with consumers, considering their unique circumstances and treatment goals. Participate in team meetings
Crisis Intervention:
Develop crisis intervention plans related to assessments, information from the consumer, and other natural resources.
Advocacy:
Advocate for clients' needs within the ACT team and with external agencies. Work with external providers to ensure that accommodations and support services are tailored to clients' mental health and recovery needs.
Collaboration:
Work closely with other ACT team members, including clinicians, housing specialists, and vocational specialists, to develop PCP goals. Participate in team meetings to share updates on clients' housing status and address any barriers.
Documentation:
Maintain accurate and timely documentation of client interactions. Ensure compliance with confidentiality and documentation standards.
Qualifications:
Bachelor's or Master's degree in social work, psychology, counseling, or a related field. Relevant experience working with individuals with severe mental health and behavioral health challenges Knowledge of local resources and programs supporting individuals with mental health needs. Strong interpersonal and communication skills. Must possess the ability to work collaboratively within a multidisciplinary team. Understanding of trauma-informed care and recovery-oriented principles.
