OVERVIEW
The Case Manager will assist clients with substance use disorders and mental health issues to gain access to rehabilitation programs and other community services. Case Managers meet with clients and their families, connect clients with professional services, and maintain consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed. They will provide SUD case management and QMHS to clients by listening to clients' concerns and providing counseling or intervention as required.
The Schedule will be:
Sunday-Thursday , or Tuesday-Saturday
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES
Provide and document case management services, therapeutic behavioral health services, to clients who are recovering from SUD. Provide additional support to the clinical and administrative team in all twelve core SUD functions (screening, intake, assessments, individual & group counseling, treatment planning, discharge planning, crisis management). Coordinate care with internal clinical staff and external providers. Monitor client progress and advocate for client needs. Document all services in accordance with agency, state, and Medicaid requirements. Participate in team meetings, supervision, and ongoing training. Maintain appropriate licensure and adhere to ethical and professional standards. Maintain 25 hours of productivity out of a 40-hour workweek. Perform other duties as assigned.
EDUCATION, TRAINING & EXPERIENCE REQUIREMENTS
High School diploma or equivalent. Current CDCA certification in the state of Ohio. Experience providing group counseling services or support services to individuals with substance use or co-occurring disorders. QMHS qualification preferred or 3 years' experience in the behavioral health field . Strong communication, organization, and problem-solving skills. Valid driver's license and reliable transportation.
