Mental Health Case Manager Crisis Center of Comal County | New Braunfels, TX Full-Time | Exempt Make a Difference When It Matters Most Are you passionate about mental health, trauma-informed care, and helping people find stability during some of the most difficult moments of their lives? The Crisis Center of Comal County (CCCC) is seeking a compassionate, emotionally grounded, and resourceful Mental Health Case Manager to join our team. This position offers a unique opportunity to work alongside survivors of domestic violence, sexual assault, homelessness, and other crises, helping them navigate mental health challenges, access appropriate resources, and move toward greater safety and stability. The Mental Health Case Manager works throughout the shelter program, responding to clients based on their mental health and emotional support needs. You will be an important resource for both clients and the shelter team—providing crisis support, helping clients navigate behavioral health services, and strengthening continuity of care. What You'll Do As our Mental Health Case Manager, you will: Provide trauma-informed emotional support, crisis intervention, de-escalation, grounding, and stabilization to clients experiencing distress or crisis. Assess immediate mental health and emotional support needs and help determine appropriate next steps, resources, and referrals. Work with clients to identify coping strategies, strengths, support systems, and resources that promote emotional stability and safety. Develop and update safety plans based on each client's individual circumstances and needs. Recognize when a client may need a higher level of care and help facilitate connections to crisis, emergency, medical, behavioral health, substance use, or other specialized services. Help clients understand and navigate available mental health services, treatment options, crisis resources, and community-based supports. Address practical barriers to accessing care, including transportation, appointments, insurance and benefits, communication with providers, and other challenges. Coordinate referrals and follow up to help ensure clients successfully connect with needed services. Collaborate closely with Case Managers, Case Aides, clinical staff, advocates, shelter leadership, behavioral health professionals, and community partners. Participate in multidisciplinary case staffing and case review meetings. Help clients develop transition plans that include appropriate mental health resources, crisis contacts, providers, and continuity-of-care supports when leaving shelter services. Maintain timely, accurate, and confidential documentation of services, referrals, safety planning, and interventions. Participate in the agency's Sexual Assault Crisis Response on-call rotation, as assigned. Are comfortable collaborating across disciplines and building relationships with community providers. Are organized enough to balance immediate crises, referrals, follow-up, documentation, and ongoing client needs. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, Behavioral Health, or a related social services field required; Master's degree preferred. QMHP-CS preferred. One to two years of experience in mental health services, case management, crisis intervention, behavioral health, victim advocacy, homelessness services, or a related human services setting preferred. Experience supporting individuals impacted by mental health concerns, trauma, homelessness, domestic violence, sexual assault, emotional distress, or other crisis situations preferred. Knowledge of trauma-informed care, crisis intervention, behavioral health resources, substance use services, and community-based support systems. Strong communication, interpersonal, documentation, organizational, and time-management skills. Ability to exercise sound judgment and maintain composure in rapidly changing or emotionally challenging situations. Ability to work both independently and as part of a multidisciplinary team. Proficiency with Microsoft Office 365 and electronic client-management systems. Bilingual English/Spanish skills are preferred. Valid Texas driver's license, proof of auto liability insurance, and reliable transportation required. Must successfully complete required background checks, fingerprinting, drug screening, and driver's record check. First Aid and CPR certification required and may be completed after hire with agency guidance.
