Job Description

Part-Time Hospice Chaplain Devotion Hospice, LLC | Highland, Michigan Devotion Hospice is seeking a compassionate and dependable Part-Time Hospice Chaplain to provide spiritual and emotional support to hospice patients, families, and caregivers. Responsibilities Complete spiritual assessments and identify patient and family spiritual needs, beliefs, preferences, and goals Develop and update individualized spiritual plans of care Provide routine, PRN, and end-of-life spiritual support Respect and support patients of all faiths, beliefs, cultures, and those who do not identify with a particular faith Provide support and guidance to families and caregivers throughout the hospice journey Participate in interdisciplinary group (IDG) meetings and contribute to the interdisciplinary plan of care Coordinate with community clergy or other spiritual resources when requested by the patient or family Complete visit documentation, care plan updates, and other required documentation accurately and timely Provide bereavement support as appropriate Collaborate closely with nurses, social workers, aides, physicians, and other members of the hospice team Qualifications Education, training, and/or experience appropriate to providing spiritual care in a hospice setting Hospice, healthcare, chaplaincy, pastoral care, or end-of-life experience preferred Ability to provide nonjudgmental spiritual support consistent with each patient's individual beliefs and choices Strong communication and interpersonal skills Reliable transportation and ability to travel to patient homes, assisted living communities, and skilled nursing facilities Ability to work independently while functioning as an active member of the interdisciplinary hospice team Must meet all applicable hospice personnel, background check, and employment requirements Position Part-Time | Flexible Daytime Schedule This is a community-based position with travel throughout our service area. Pay:
$28.00 - $35.00 per hour Expected hours: No less than 10.0 per week
