Position Summary Proactive Hospice Care is seeking a compassionate and experienced Hospice Chaplain & Bereavement Coordinator for a part-time, contingent, per-diem position. This role provides spiritual care, emotional support, and bereavement services to hospice patients and their families in alignment with their individual beliefs and preferences. This position is paid per visit, with compensation for initial spiritual assessments and follow-up (re-visit) encounters. Key Responsibilities Provide spiritual and emotional support to hospice patients, families, and caregivers Conduct initial spiritual assessments and ongoing re-visits as indicated Develop and contribute to individualized spiritual care plans Facilitate prayer, meditation, rituals, or spiritual practices as requested Respect and support diverse religious, spiritual, and cultural beliefs Collaborate with the Interdisciplinary Group (IDG) to address spiritual and emotional needs Participate in IDG meetings and care planning discussions Provide and coordinate bereavement support, including follow-up calls, visits, and memorial services Assist families during the grieving process per hospice bereavement guidelines Complete timely and accurate documentation in compliance with hospice regulations and agency policies Qualifications Bachelor or Master's degree in Divinity, Theology, Pastoral Counseling, or related field preferred Ordination or endorsement by a recognized faith tradition preferred Minimum 2 years of experience providing spiritual care (hospice or healthcare setting preferred) Knowledge of Kinnser/Well Sky EMR preferred Clinical Pastoral Education (CPE) certification a plus Strong interpersonal, communication, and listening skills Ability to work independently and as part of an interdisciplinary team Demonstrated ability to support individuals from diverse spiritual, religious, and cultural backgrounds Compensation & Schedule Per-Diem / Per-Visit Pay Paid per Initial Assessment and Re-Visit Flexible, visit-based scheduling Ideal for Chaplains seeking part-time, PRN, or supplemental work Work Environment Patient homes, facilities, and office or occasional memorial services Local travel required Why Join Proactive Hospice Care Mission-driven, compassionate work Flexible schedule with per-diem compensation Supportive and collaborative interdisciplinary team Opportunity to make a meaningful impact on patients and families How to Apply If you feel called to provide spiritual care and bereavement support during end-of-life journeys, we encourage you to apply.
