Job Description

About the Role The CFS Direct Support Coordinator provides Coordinated Family Support services to adults with developmental disabilities aged 18 and over who are consumers of the regional center and have chosen to live in their family home. The Coordinator is the family's consistent point of contact and delivers services primarily in the family home. Responsibilities Supports and Navigation Identifies with the consumer and family the supports required to live successfully in the family home, revisiting as circumstances change Assists the consumer and family in navigating service delivery systems and overcoming barriers to access Provides information about the consumer's diagnosis and identified supports, in the family's primary language Assists with scheduling appointments and services, and confirms afterward that they occurred Identifies and supports use of available transportation options Continuity and Planning Coordinates consistent training across providers serving the consumer Identifies backup providers in advance and delivers backup support directly when a support plan fails Conducts futures planning with the consumer and family, including planning for aging caregivers Trains and coaches the consumer in skills that increase independence at home and in the community Documentation and Reporting Creates a service record for every contact (date, duration, location, participants, support provided, outcome) Completes records contemporaneously; late entries are dated the day written and identified as late — records are never created or dated retroactively Contributes to the annual assessment of the consumer's continued need for service Prepares quarterly progress reports for Supervisor review and approval Reports special incidents to the Supervisor immediately and completes Special Incident Reports within required timelines Fulfills mandated reporter obligations Coordination Works with the consumer's regional center service coordinator Notifies the Supervisor of material changes in the consumer's circumstances Qualifications Required Associate's-level degree in a human services field; OR three years of experience in the developmental disabilities service delivery system (degree and experience routes are equivalent) Ability to communicate in the primary language of the consumer and family Knowledge of the Regional Center system Preferred Experience supporting adults with developmental disabilities in a home or community setting Working knowledge of behavioral health services, In-Home Supportive Services, Area Agencies on Aging, Social Security, and adult education or employment services Experience in person-centered or futures planning Bilingual capability in a language spoken by families served Boundaries of the Role Coordinated Family Support services may not be provided by a person who resides in the consumer's home These services do not replace or duplicate other regional center services the consumer is authorized to receive Screening Requirements Verification of required qualifications, documented in personnel file before consumer assignment Criminal background clearance before direct consumer contact, maintained throughout employment Confirmation of no listing on the OIG List of Excluded Individuals and Entities or the DHCS Medi-Cal Suspended and Ineligible Provider List Employment and reference verification Valid California driver's license, reliable transportation, and current vehicle insurance Tuberculosis clearance and any additional required health screening Training Vendor Orientation, completed on or before first day of service delivery Accounting Orientation Special Incident Reporting training (reportable events and Title 17 timelines) Mandated Reporter training, with periodic refresher Within 30 days of engagement: training on relevant service delivery systems, including behavioral health services and the Area Agencies on Aging network Ongoing training per Title 17 Sections 58651 and 58652 Working Conditions Services are delivered in consumers' family homes and community settings, with some administrative work in the office. Requires travel within the service area, effective communication in person/by phone/in writing, and the ability to sit, stand, walk, and occasionally lift up to 20 pounds. Pay:
$24.00 per hour Expected hours: 20.0 - 40.0 per week
