Job Description

Join Our Team for our long-term care nursing home We are seeking a compassionate, organized, and dedicated Long TGerm Care Social Services Assistant to support the psychosocial well-being of residents in our skilled nursing facility. Working closely with the Social Services Director, interdisciplinary team, residents, and families, the Social Services Assistant helps ensure residents receive the emotional, social, and community support needed to enhance their quality of life. Have experiance using Point Click Care, Discharge planning, Care Planing, MDS and flexable hours to meet the resdients needs. Position Summary The Social Services Assistant assists in providing social services to residents, helping them adjust to facility living, maintain emotional well-being, access resources, and participate in discharge planning. This position plays a vital role in advocating for residents' rights and supporting residents and families throughout their stay. Essential Responsibilities Assist the Social Services Director in assessing residents' psychosocial needs. Support residents and families with adjustment, coping, and transition issues. Participate in care plan meetings and interdisciplinary team discussions. Assist with discharge planning and coordination of community resources. Maintain accurate and timely documentation in accordance with facility policies and state and federal regulations. Provide information regarding resident rights, advance directives, and available services. Help coordinate resident and family meetings as needed. Support activities that promote residents' emotional and social well-being. Monitor residents for psychosocial concerns and report findings to appropriate team members. Assist with referrals to community agencies and support services. Maintain confidentiality and comply with HIPAA regulations. Qualifications Associate's degree in Social Work, Human Services, Psychology, or a related field preferred. Experience in long-term care, skilled nursing, healthcare, or human services preferred. Knowledge of resident rights, discharge planning, and community resources is a plus. Strong communication, interpersonal, and organizational skills. Ability to work effectively with residents, families, and interdisciplinary team members. Proficiency with electronic medical records and Microsoft Office applications preferred. Ability to occasionally lift up to 25 pounds. Ability to travel throughout the facility to meet with residents and staff.
