BIANJ is expanding its Support Coordination Department and looking for a creative, personable, and resourceful Support Coordinator to join our team! You'll work closely with a supportive, dedicated team with accessible and experienced supervision, and have the full backing of the entire organization to ensure your success. This position is primarily home-based, with one required office day per month (in North Brunswick) after onboarding and training. This role will have the majority of cases in Monmouth county and would require a candidate living in that area.

Key Responsibilities:

Provide support coordination/case management for individuals eligible for DDD services. Develop and monitor personalized service plans for adults with developmental disabilities. Assist individuals in exploring and accessing community supports. Build and maintain positive relationships with families and community providers. Travel required within New Jersey; a valid NJ driver's license and reliable transportation are necessary.

Qualifications:

Bachelor's degree (any field; social services degree preferred). At least one year of experience (including volunteer work) with adults with developmental disabilities. Strong organizational skills and ability to work independently. Ability to meet deadlines and collaborate effectively with a team. Previous experience in social services preferred but not required. Valid driver's license and dependable transportation required. Must pass background checks through DDD and the Brain Injury Alliance of New Jersey. Ready to make a real impact? From $43,000.00 per year
