Director of Social Services Duties :

• Coordinate/plan safe and appropriate discharges with the resident and other responsible parties.
• Arrange post-discharge services
• Serve as a primary resource for residents and families regarding psychosocial concerns
• Facilitate communication between residents, family, and facility departments
• Assist with conflict resolution regarding resident/family concerns
• Complete required assessments withing required timeframes
• Identify residents' psychosocial, behavioral, emotional, and discharge-related needs
• Complete applicable MDS and supporting documentations within deadlines
• Participate in care plan meetings to discuss the necessities in the resident's plan of care
• Continuously follow up with residents to ensure interventions/solutions have been addressed.
• Collaborate with other departments and community organizations to coordinate services and resources
• Ensure compliance with all relevant regulations and standards
• Monitor program effectiveness and make necessary adjustments to improve outcomes
• Maintain accurate records and documentation of client interactions and program activities

Requirements :

• Proven experience in a leadership role within social services or a related field
• Strong knowledge of behavioral health principles and practices
• Ability to effectively communicate with diverse populations, both verbally and in writing
• Strong organizational skills and attention to detail
