Job Description
Director of Social Services Duties :
- Coordinate/plan safe and appropriate discharges with the resident and other responsible parties.
- Arrange post-discharge services
- Serve as a primary resource for residents and families regarding psychosocial concerns
- Facilitate communication between residents, family, and facility departments
- Assist with conflict resolution regarding resident/family concerns
- Complete required assessments withing required timeframes
- Identify residents' psychosocial, behavioral, emotional, and discharge-related needs
- Complete applicable MDS and supporting documentations within deadlines
- Participate in care plan meetings to discuss the necessities in the resident's plan of care
- Continuously follow up with residents to ensure interventions/solutions have been addressed.
- Collaborate with other departments and community organizations to coordinate services and resources
- Ensure compliance with all relevant regulations and standards
- Monitor program effectiveness and make necessary adjustments to improve outcomes
- Maintain accurate records and documentation of client interactions and program activities
Requirements :
- Proven experience in a leadership role within social services or a related field
- Strong knowledge of behavioral health principles and practices
- Ability to effectively communicate with diverse populations, both verbally and in writing
- Strong organizational skills and attention to detail
