Job Description

Job Summary Life Care Center of Michigan City is seeking a dedicated and experienced Social Services Director to lead our skilled nursing facility's social services department. The ideal candidate will possess a comprehensive understanding of clinical social work practices, patient care coordination, and community resource management. As the Social Services Director, you will oversee the delivery of psychosocial support, discharge planning, and case management services to enhance patient well-being and ensure compliance with healthcare regulations. This role offers an opportunity to make a meaningful impact on residents' quality of life while leading a compassionate and professional team. Duties Develop and implement individualized care plans that address the psychosocial needs of residents, including mental health counseling, behavioral therapy, and end-of-life care. Coordinate with interdisciplinary teams to facilitate comprehensive patient assessments, discharge planning, and clinical documentation in accordance with HIPAA and social work practice standards. Lead efforts in community resource coordination to connect residents with appropriate support services such as hospice care, addiction counseling, or specialized therapies like applied behavior analysis. Supervise social work staff, providing training on clinical ethical guidelines, crisis intervention techniques, and documentation procedures. Conduct intake assessments for new residents, ensuring accurate medical records management and adherence to Medicare and Medicaid requirements. Manage data collection related to patient outcomes and social work interventions to support clinical research initiatives and quality improvement projects. Ensure compliance with all regulatory standards including
HIPAA, ICD
coding (ICD-10), CPT coding, and other healthcare documentation protocols. Promote a culture of compassionate care by fostering teamwork, cultural sensitivity, and client-centered approaches across the facility. Qualifications Proven experience in medical social work within a skilled nursing or long-term care setting; inpatient experience preferred. Strong knowledge of medical terminology, clinical counseling techniques, and behavioral health practices such as cognitive behavioral therapy and motivational interviewing. Familiarity with hospice & palliative medicine, dementia care (including Alzheimer's), PTSD care, and working with individuals with developmental disabilities or autism spectrum disorder. Demonstrated ability to manage casework involving complex discharge planning, crisis management, and community resource linkage. Excellent communication skills for client interaction, interprofessional collaboration, and staff supervision. Knowledge of healthcare regulations including HIPAA compliance, Medicare/Medicaid policies, ICD-9/ICD-10 coding systems, and EMR systems. Certification in basic life support (BLS) or CPR is highly desirable; additional certifications in clinical research or addiction counseling are advantageous. Bachelor's degree in social work (BSW) or related field required; master's degree preferred along with licensure as a Licensed Clinical Social Worker (LCSW).
