Alliance Health at Coleman is a not-for-profit 45-bed skilled nursing center in Northborough, MA. We are looking for full-time Social Worker. Responsible to provide social work services and support to residents and their families. Responsible for the completion of required documentation of psychosocial visits and assessments.

Education required:

Bachelor's degree in Social Work or related field Professional licensure required: Massachusetts LCSW, or LICSW. Long Term Care Social Worker must communicate effectively with residents, families, visitors, and interdisciplinary team to support the emotional, mental, and clinical well-being of residents. Familiar with Medicare and Medicaid regulations and procedures. Plans and facilitates the weekly Care Plan meetings. Completes the PASARR requirements in accordance with regulations.
