South Mountain Rehab Center, a 89-bed facility in historic Boonsboro, Maryland, is seeking a dedicated and compassionate Social Services Director to join our team. As a key member of our leadership team, you will be responsible for planning, developing, organizing, implementing, evaluating, and directing social services programs, while ensuring excellent care and services to our residents. Responsibilities:
Plan, develop, organize, implement, evaluate, and direct social services programs to meet the needs of our residents Develop, administer, and coordinate department policies and procedures to ensure compliance with federal and state regulations and professional standards Keep abreast of current laws, regulations, and professional standards, and make recommendations to the Administrator Participate in community planning related to the interests of our facility and services Perform administrative tasks, such as completing necessary forms, reports, and submitting them as required Make written and oral reports and recommendations to the Administrator and relevant department heads Handle emergencies as they arise, such as rescheduling work assignments and tasks
Specific Functions:
Orient residents and families to our facility, helping them adjust to their new environment Develop social histories and assessments for each resident Develop social care plans, identifying medically related social and emotional problems and needs Answer inquiries and provide tours to prospective families and residents Assist in pre-admission and admissions processes Interview residents and families as necessary Prepare and complete social service documentation in a timely manner Participate in resident care plan conferences and care plan team meetings Assist in arranging transportation as needed Refer residents and families to appropriate social service agencies when necessary Document residents' behavioral and psychosocial status Chair Resident and Family Councils to support these groups Why Work for
