Job Description

Job Title:

Director of Social Services Join a healthcare team committed to creating a positive experience for staff and patients.

Requirements:

College degree required. Licensed Social Worker may be required based on regulations and facility size. Skilled Nursing experience preferred.

Responsibilities:

Plans, develops, organizes, implements, evaluates, and directs the social service programs of the facility. Participates in discharge planning, development, and implementation of social care plans and resident assessments. Interviews residents and families as necessary to identify medically related social and emotional needs.
