2989949371-2 Apply for this job now Job Description Humana is seeking a Case Manager to support members across Green Lake, Marquette, Adams, and Waushara counties. In this role, you will conduct comprehensive assessments, develop individualized care plans, and coordinate medical, behavioral, and social services to improve members' health outcomes. You will collaborate closely with physicians, nurses, social workers, and community partners to connect members with the right care and resources, while educating them on benefits and self-management strategies. Working within Humana's inclusive, member-focused culture, you'll document cases in electronic systems, track progress, and ensure compliance with quality and regulatory standards. Responsibilities Manage care coordination for members across Green Lake, Marquette, Adams, and Waushara counties Conduct comprehensive assessments to identify medical, behavioral, and social needs Develop and monitor individualized care plans focused on improved health outcomes Collaborate with physicians, nurses, social workers, and community partners Educate members and families on benefits, resources, and self-management strategies Track progress, document cases, and maintain accurate electronic health records Advocate for member needs and address barriers to care (transportation, housing, access) Monitor utilization and escalate complex cases to clinical leadership Ensure compliance with Humana policies, quality standards, and regulatory requirements Participate in team meetings, training, and continuous improvement initiatives Required Skills Case management Care coordination Member assessment Care plan development Electronic health records (EHR) documentation Knowledge of Medicaid/Medicare Community resource navigation Data and outcomes tracking Interdisciplinary collaboration Regulatory and compliance knowledge
