Position Summary:

The Case Manager provides professional case management, service coordination, assessment, advocacy, and supportive intervention to children, adults, families, and vulnerable populations receiving services through Florence County Human Services. The position assists individuals in identifying strengths, assessing needs, coordinating community resources, and developing person-centered plans that promote safety, independence, permanency, recovery, and overall well-being. Due to the rural nature of Florence County, the Case Manager serves a diverse population and is expected to work collaboratively across multiple human service programs while maintaining compliance with applicable Wisconsin Statutes, Administrative Code, County policies, Medicaid requirements, and departmental standards.

Essential Duties and Responsibilities:

Case Management Conduct comprehensive biopsychosocial, functional, and environmental assessments. Determine eligibility for county, state, and federally funded programs. Develop individualized service plans with measurable goals. Monitor progress toward identified goals. Coordinate services among multiple providers and agencies. Complete reassessments according to program requirements. Maintain appropriate caseloads as assigned. Client Services Provide case management services including: Intake and assessment Resource identification

• Referral coordination Advocacy Crisis intervention Safety planning Monitoring service effectiveness Family engagement Community integration Independent living skill development Documentation Maintain accurate documentation including: Assessments Service plans Progress notes Case summaries Court reports Incident reports Required state reporting Electronic health record documentation Medicaid documentation (when applicable) Documentation shall be completed within departmental timelines and meet audit standards. Community Collaboration Develop collaborative working relationships with: Schools Health care providers Behavioral health providers Law enforcement Courts District Attorney Corporation Counsel Public Health Aging & Disability Resource Centers Tribal agencies Foster parents Residential providers Community organizations State agencies Home and Community Visits Conduct scheduled and unscheduled visits including: Home visits School visits Hospital visits Community-based meetings Residential facility visits Travel throughout Florence County and neighboring counties is required.

Crisis Response Assist with:

Emergency situations Child safety concerns Adult protection concerns Mental health crises Substance use emergencies Coordination with crisis response services Emergency placement planning Court Responsibilities Prepare and present: Court reports Affidavits Case summaries Testimony Permanency documentation Protective service recommendations Appear in court as required. Resource Coordination Assist clients in accessing: Medical care Behavioral health services Housing Employment Financial assistance Transportation Educational services Vocational services Food assistance Child care Community supports Compliance Maintain compliance with: Wisconsin Statutes Wisconsin Administrative Code County policies Medicaid requirements

Equivalent education and experience may be considered where permitted by program requirements. Case managers performing Medicaid-funded assessments and case planning must meet applicable Wisconsin qualification requirements.
