Job Description

Job Purpose:

Under the Assistant Director's direction, the housing navigator provides homeless prevention case management services to Galilee Center clients seeking assistance. The program offers short-term support to individuals and families at imminent risk of homelessness, working to keep them out of shelter through housing stabilization and alternative housing solutions. This role is critical in reducing shelter use and promoting long-term housing stability for at

  • risk individuals and families.
Essential Duties:

Implement best practices for prevention and diversion to reduce/eliminate the need for shelter. Complete intakes for all persons identified as prevention/diversion candidates. Conduct prevention and diversion assessments to identify alternatives to shelter. Provide customer service in a friendly, respectful manner. Build relationships with landlords, social service agencies, and other community partners. Develop a professional network of landlords to secure housing for qualified clients. Identify immediate client needs from intakes and assessments. Mediate and resolve conflicts with friends, family, landlords, and others to secure safe housing. Provide initial and ongoing assessments and develop/implement service plans. Link clients to community resources and support them in applying for public assistance (e.g., Medicaid, food stamps, TANF). Advocate for clients with landlords, employers, and agencies. Maintain regular contact with the caseload and participate in client staffing. Assist with client transportation as needed. Complete ongoing self-training, reporting findings, and suggestions to the supervisor. Provide Crisis and/or short
