Job Description
Health Care Resource Centers seeks a Full Time Substance Abuse Counselor to provide patient-centered care in an outpatient opioid treatment program. Responsibilities include conducting assessments, facilitating individual and group counseling, developing treatment plans, documenting progress, and coordinating care with an interdisciplinary team. The counselor ensures regulatory compliance, promotes evidence-based, medication-assisted treatment, and supports patients through all stages of recovery. Responsibilities Provide individual and group counseling to patients with opioid use disorder Conduct biopsychosocial assessments and ongoing clinical evaluations Develop, implement, and update individualized treatment plans Collaborate with physicians, nurses, and other team members on patient care Maintain accurate, timely clinical documentation and records Support medication-assisted treatment and educate patients on recovery strategies Ensure compliance with federal, state, and program regulations and policies Participate in case conferences, staff meetings, and quality improvement initiatives Coordinate referrals to community resources and higher levels of care as needed Promote a respectful, trauma-informed, patient-centered care environment Required Skills Substance abuse counseling Clinical assessment and diagnosis Treatment planning and documentation Group counseling facilitation Motivational interviewing Trauma-informed care Knowledge of medication-assisted treatment (MAT) Electronic health record (EHR) documentation Crisis intervention Regulatory and compliance knowledge for opioid treatment programs
