Job Description

The Referral Tracker is responsible for maintaining ongoing tracking and appropriate documentation on referral status. Connects with offices/ facilities and requests the notes/ reports for continuity of care and reaches out to patients to reschedule appointment as needed. Requires a high school diploma or equivalent. A minimum of 2 years of experience in a medical office preferred. A minimum of one (1) year experience as a Referral Specialist with ESRH required. Individual must be professional, self-motivated and have strong attention to detail, communication, teamwork, and computer skills. Must demonstrate excellent internal and external customer service skills. Provide coverage at other centers, or for other providers, on short notice when needed. Certification as an Interpreter is preferred and ability to provide bilingual patient care is a plus. Review open referrals and search for related reports that may already be in the patients' medical records. Address referral if report is found. Communicate with specialists/ imaging facilities to determine patient compliance with attending appointment. Document compliance in notes tab of the referral. Request consultations/ imaging reports for appointments attended. Continue to communicate with specialist/ imaging facilities as needed until all reports are received. Address referral once the report is received. Inform Referral Coordinator of issues related to communications with specialist/imaging facilities. Communicate with patients who did not attend scheduled appointment to determine if referral is still warranted and direct the patient to reschedule if still needed. Follow the Referral Initiation, Scheduling and Tracking Procedure (C PRO 80) for addressing referrals that are no longer wanted or if unbale to reach patient. Assume the duties of a Referral Specialist when needed. Other duties and tasks as directed by the Referral Coordinator.
ADDITIONAL DUTIES ASSIGNED TO DESIGNATED BILINGUAL STAFF
If qualified, perform interpretation/translation services for ESRHS centers.
