2996662143-2 Apply for this job now Job Description Hartford Healthcare seeks a Hospice Quality Assurance Performance Improvement Coordinator to lead quality, compliance, and performance initiatives across our hospice programs. In this RN role, you will monitor clinical quality metrics, conduct chart audits, and ensure adherence to regulatory and accreditation standards. You will analyze data, identify trends, and implement evidence based performance improvement plans in collaboration with an interdisciplinary team. The coordinator will educate staff on best practices, support survey readiness, and champion a culture of safety, compassion, and continuous improvement for patients and families. Responsibilities Monitor hospice quality metrics and patient outcomes to ensure high standards of care Conduct regular chart reviews and audits for regulatory and accreditation compliance Analyze data to identify trends, gaps, and opportunities for performance improvement Lead quality and performance improvement projects using evidence-based methodologies Develop and update policies, procedures, and clinical practice guidelines Educate and mentor hospice staff on quality, documentation, and best practices Collaborate with interdisciplinary teams to improve care coordination and patient experience Prepare for and support internal and external surveys and inspections Report quality findings and improvement plans to leadership and key stakeholders Promote a culture of safety, accountability, and continuous improvement Required Skills Hospice clinical knowledge Quality assurance and auditing Performance improvement methodologies (PDSA, Lean, Six Sigma) Healthcare regulatory and accreditation standards (CMS, Joint Commission) Data collection, analysis, and reporting Electronic health records (EHR) systems Clinical documentation standards Policy and procedure development Staff training and education Interdisciplinary team collaboration
