Program Coordinator Flint, MI 48503 Job Description In partnership with the Genesee County Office of Senior Services, Family Service Agency offers a program available to Genesee County Seniors unlike anything before: Transport from their home to their medical appointment Liaison support at and during their senior's medical appointment Transport to the pharmacy or grocery for dietary needs following the appointment Follow-up appointment transportation Out-of-county specialist transportation
PROGRAM DRIVERS WILL ACT AS LIAISONS TO THE SENIOR IN NEED. THEIR DUTY IS TO GREET THE SENIOR AT THEIR DOOR, ASSIST THEM TO THE VEHICLE, AND ACCOMPANY THEM TO THE DOCTOR'S APPOINTMENT AND WAIT FOR THE APPOINTMENT TO BE COMPLETED, THEN TRANSPORTED TO THE PHARMACY OR GROCERY STORE SHOULD THE APPOINTMENT REQUIRE SUCH NEEDS, AND FINALLY RETURNED HOME. ROLE AND RESPONSIBILITIES
Abide by assigned schedules on a daily basis Assist Genesee County Seniors to and from their medical appointments Collaborate and take instruction from the program coordinator to complete daily tasks and routes Communicate vehicle maintenance needs to the program coordinator Be able to perform the job between 7:30 AM to 6:00 PM. Be able to commit to 15-28 hours per week.

PROGRAM NARRATIVE PLEASE READ THIS THOUROUGHLY PRIOR TO APPLYING FOR THIS POSITION!
Program Summary The proposed Senior Transportation Program is designed for all senior residents, 60 years old or older, residing in Genesee County. The program is designed to provide free transportation to seniors in the community who cannot drive or do not have proper funding to use public/commercial transport. Seniors in need will contact the FSA Program coordinator for assessment and arrangement of their ride within Genesee County. All participants must reside in Genesee County and their appointment must be in Genesee County. The program will offer rides utilizing two different vehicles. The participating seniors can utilize an SUV with a wheelchair lift or a small SUV if they do not require a wheelchair lift. All marketing materials and program brochures will include the Genesee County Seal and Senior Millage recognition. The services offered through this program will operate out of Family Service Agency's building in Flint, MI. All rides will take place in Genesee County. Program Staff Duties and Requirements The Senior Transportation Service program will operate under a program coordinator overseeing three part-time drivers for two separate vehicles. The Program Coordinator will be responsible for the following: 1. Assessments 2. Community connections and outreach 3. Route/Schedules 4. Driver management 5. OSS Liaison for program needs/requirement 6. Vehicle maintenance checklists and coordination (oil changes, tire rotations, fluid checks, etc.) 7. Mileage records Program drivers will be responsible for the following: 1. Transport of the senior 2. Monitoring the condition of the vehicle In addition to the Program Coordinator will be three program drivers. The drivers will fulfill their given schedule on a daily basis. Drivers will be responsible for the transport and any/all assistance required by the senior (i.e. getting from their door to the vehicle, and exiting the vehicle). All program drivers must obtain their Chauffer's certification within 90 days of hire. The following shall be conducted on each program staff annually (FSA is to cover the costs of all items except a valid driver's license): TB Test ICHAT Background check MI Sex offender registry National Sex offender registry Driving record history Chauffer's license Valid driver's license Driving History check Senior Program Eligibility Requirements All participants must meet the following criteria: Genesee County resident Appointment is in Genesee County (must be medical) No able-bodied adult in the home available to provide transport Unable to transport oneself Unable to afford or obtain own vehicle 60 years old or better
