Job Description
Job Description We are seeking a Population Health Management (PHM) Business Analyst Subject Matter Expert to serve as the primary resource for internal teams and vendor partners on population health programs, data sources, business processes, and implementation planning. This individual will provide strategic guidance on scope, timelines, business requirements, and healthcare data while translating complex concepts for both technical and non-technical stakeholders. - Serve as the Population Health Management SME, participating in 5+ meetings daily with internal stakeholders, vendors, and project teams.
- Provide guidance on project scope, timelines, business requirements, dependencies, and implementation strategies.
- Document and maintain business rules, workflows, process documentation, one-pagers, policy guides, and supporting materials.
- Answer functional and data-related questions regarding population health programs, healthcare operations, and reporting requirements.
- Act as a liaison between business and technical teams, translating complex healthcare concepts into actionable requirements.
- Educate and coach teams on Population Health Management initiatives, data sources, and business processes.
- Lead discussions around healthcare data assets including claims, EMR/EHR, ECM/CS, CalFresh/nutrition, correctional/JI data, and other population health data domains.
- Review requirements and provide recommendations to vendors and implementation teams to ensure alignment with business objectives.
- Facilitate organizational meetings, drive decisions, and ensure stakeholders remain aligned throughout project lifecycles.
- Support onboarding and knowledge transfer activities, helping teams ramp up and gain PHM domain expertise.

https://insightglobal.com/workforce-privacy-policy/. Skills and Requirements- 3+ years of Business Analyst experience supporting healthcare, managed care, Medicaid, or Population Health Management initiatives.
- Deep subject matter expertise in Population Health Management programs and healthcare operations.
- Strong experience working with healthcare data sources including claims, EMR/EHR, and other clinical, social determinants, and community-based datasets.
- Proven ability to document business rules, requirements, process flows, and operational procedures.
- Exceptional communication and presentation skills with the ability to explain complex concepts to both technical and non-technical audiences.
- Experience working directly with vendors, implementation partners, and cross-functional business teams.
- Ability to lead discussions, influence stakeholders, and provide consultative guidance without direct authority.
- Strong facilitation skills with experience organizing and leading large stakeholder meetings.
- Professional, polished, and persuasive communication style with a customer-service mindset.
- Passion for healthcare data and using information to drive population health outcomes and program success.
