Job Description
For more than 30 years, Med-Scribe, Inc. has been connecting highly qualified clinicians with top-tier healthcare opportunities throughout New York State. We are partnered with a prominent multinational organization committed to strengthening communities and enhancing quality of life by linking individuals to vital government health and human services. We are seeking experienced clinicians to join our expanding UAS Home Care Project- an opportunity to make valuable professional judgments, work independently, and build impactful connections with patients while maintaining flexibility in your schedule. In this contract role, you will leverage your clinical expertise to independently review and evaluate completed UAS reassessments, determining the appropriateness of long-term care services in alignment with Medicare and Medicaid guidelines. Appointments are in-person with complex care individuals who are applying for these Managed Long Term Care (MLTC) services and take place in patients' homes, healthcare facilities, or other settings in the community. Your insights will directly support high-quality, patient-centered care decisions determining whether the individual can qualify for services in their home or require a supportive facility. Each evaluation takes approximately 1.5 hours including 30 minutes each for preparation, an in-person consumer interview, and determination submission, with additional compensation available for extended time.

10:00 a.m. to 6:00 p.m. Travel is essential for this role! Appointments are scheduled within your selected county or counties. Candidates covering multiple counties may have access to increased hours. Please choose one full county to be considered.Current openings include:
Chemung Tompkins Tioga Compensation is $100 per hour. Contractors are compensated for hours worked, plus partial compensation for interrupted assessments. These roles are 1099 contractor positions and do not include benefits.Minimum Qualifications:
- NYS Certified Nurse Practitioner License plus 3600 hours of experience OR
- NYS Certified Physician Assistant License
- An active NYS Medicaid number
- 2 years of prior experience as a Nurse Practitioner or Physician Assistant within home care, geriatrics, or community health populations is required
