Utilizing your strong understanding of all the Ryan White Services, you will know and understand the contract deliverables, Standards of Care, and goals of both the program and the patients served. You will collaborate with the AHF Healthcare Team, including Providers, the Practice Manager, the Regional Director, and your contract manager so that your team understands the operational process and deliverables to provide guidance, resources, and access to care for all patients. You will provide guidance and operational assistance to the team and monitor services entered into the Care Management Information system. You will ensure that the programs under your oversight are audit-ready and your team has the understanding, training, and tools to implement your operational goals. Your 1:1 meeting with your team will provide an understanding of your team's performance, address any challenges, and work together to resolve. In this way, you will support the team's ability to help their patients achieve their healthcare goals. A Manager of Care Management enhances the reputation of AHF by providing summaries of program operations to the Regional Manager of Care Management, you are collaborating creatively and addressing contract expectations to meet program deliverables and improve processes.
