From $65,000 per year + Performance Bonuses About Avenora Elmhurst Avenora Elmhurst is a skilled nursing and rehabilitation facility in Elmhurst, Illinois, providing skilled nursing, short-term rehabilitation, long-term care, and other specialized services. We are seeking an experienced, energetic, and results-oriented Admissions Director to lead our admissions process, develop strong referral relationships, and help grow and maintain our resident census. This position is ideal for someone who understands skilled nursing admissions, enjoys building relationships with hospitals and healthcare professionals, and is motivated by measurable results. Responsibilities The Admissions Director will be responsible for:

• Managing the complete admissions and referral process
• Responding promptly to hospital and community referrals
• Reviewing potential admissions and coordinating clinical acceptance with the appropriate clinical team
• Communicating with hospital case managers, social workers, physicians, families, and other referral sources
• Developing and maintaining strong relationships with hospitals, rehabilitation centers, physician offices, assisted living facilities, and community referral sources
• Conducting facility tours for prospective residents and families
• Following up consistently on pending referrals
• Coordinating insurance verification and authorization requirements
• Working closely with Nursing, Administration, Therapy, Social Services, and other departments to facilitate smooth admissions
• Monitoring referral volume, acceptance rates, conversion rates, admissions, and census
• Identifying opportunities to increase Medicare, Medicare Advantage, managed-care, Medicaid, private-pay, and other appropriate admissions
• Participating in daily census and admissions meetings
• Maintaining accurate admission and referral documentation
• Helping develop marketing and census-growth strategies
• Representing Avenora Elmhurst professionally within the healthcare community Qualifications Preferred qualifications include:
• Previous admissions, marketing, business development, or liaison experience in a skilled nursing facility, rehabilitation facility, hospital, assisted living facility, or other healthcare setting
• Strong knowledge of the skilled nursing admission and referral process
• Experience working with hospital case managers and social workers
• Understanding of Medicare, Medicare Advantage, Medicaid, managed care, and private-pay admissions
• Strong communication, organizational, and follow-up skills
• Ability to build and maintain professional referral relationships
• Ability to work independently and as part of an interdisciplinary team
• Strong customer-service skills when working with residents and families
• Experience with PointClickCare or similar healthcare software is preferred
• Strong understanding of HIPAA and confidentiality requirements

Compensation Base Salary:

Starting at $65,000 per year Avenora Elmhurst also offers a performance-based bonus program that provides significant additional earning potential based on measurable results, including:

• Census growth
• Sustained average daily census
• Monthly admissions
• Qualifying skilled admissions
• Overall admissions performance High-performing candidates have the opportunity to earn substantially above the base salary.
