Job Description
Job Overview We are seeking a dedicated and compassionate Assistant Social Worker to join our multidisciplinary healthcare team. The ideal candidate will possess a strong foundation in social work practice, with a focus on providing comprehensive support to diverse patient populations. This role offers an opportunity to make a meaningful impact by assisting and advocating for individuals and families through challenging circumstances, coordinating community resources, and ensuring adherence to clinical ethical guidelines. The Assistant Social Worker will work closely with LMSW Director, healthcare providers, and community organizations to deliver patient care across various settings, including hospitals, outpatient clinics, and community-based programs. Required Bachelor's degree in Social work, Psychology, Sociology, Human Services, Mental Health. Experience work in healthcare a plus. Responsibilities FT Social Worker Assistant• needed for LTC facility with a diverse population serving short term and long term patients. BSW or related degree (psychology, sociology, behavioral health) with geriatric, behavioral health skilled or nursing facility experience preferred to work with LMSW Seeking compassionate and dynamic individual who likes working in a fast paced, busy environment. Responsibilities include an assignment of residents and: 1. Works to ensure that each resident's privacy, dignity and autonomy are protected. Works with other departments and staff to ensure that the resident's experience in the facility is in keeping with his/her needs and preferences and assures maximum attainable quality of life for each resident as much as possible. 2. Provides assistance and support to residents and their families. Assists in new admission in adjustment and on-going transition to SNF life 3. Provides necessary information to resident and families and acts as the advocated and liaison to the team. 4. Initiates and facilities care plan meetings with residents and/or families as needed. 5. As part of the interdisciplinary team , duties include assessment, MDS, social histories, patient and family advocacy, progress notes, care planning, and guidance and counselling as needed. 6. Works with residents and families on planning for discharge back to the community, or to another environment as dictated by the needs of the resident. 7. Assists residents to access community and governmental services through which the resident will receive assistance upon discharge. 8. Sets up care planning meetings with residents and families. 9. Works with resident and families on planning advanced directives, palliative care and funeral arrangements as needed. 10. Registers and assists director in working with departments on grievances registered by residents and family members. 12. Assists residents with explaining or guiding them to get financial questions or concerns addressed. 13. Acts as a liaison to federal, state and local agencies providing service to the community. 14. Maintains compliance with PASRR regulations and ensures all Level II recommendations are followed and integrated in the residents total plan of care. 15. Assists other residents and families as needed in the absence of the Director.
