The ideal candidate thrives in a high-volume, fast-paced environment and is energized by the opportunity to streamline clinical review of referral care needs, complete insurance payer verifications, and help prospective residents navigate the transition into our facility. This position requires a highly organized, technology-savvy professional with excellent communication skills, strong attention to detail, and previous nursing home admissions experience. The successful candidate will be comfortable using electronic referral management systems to efficiently track, prioritize, and manage a high volume of referrals. Key Responsibilities Manage a high volume of referrals, inquiries, and admissions requests while maintaining timely follow-up. Utilize electronic referral management systems to receive, review, track, update, and prioritize prospective admissions. Monitor referral activity and maintain accurate, up-to-date information within referral management platforms. Communicate effectively with referral sources and facility team members on prospective admits to SNF. Review referral information and insurance payers to determine clinical needs and admission eligibility. Coordinate closely with clinical, business office, and administrative teams to ensure smooth and timely admissions. Follow up consistently on referrals and prospective admissions to maximize conversion opportunities. Provide clear, compassionate, and professional communication. Assist with census development and maintaining strong referral relationships. Prioritize multiple referrals, tasks, and deadlines while maintaining accuracy and professionalism. Ensure admissions processes are completed in accordance with facility policies and applicable regulations. Qualifications Previous nursing home/skilled nursing facility admissions experience. Technology savvy with the ability to quickly learn and effectively use referral management systems and other electronic platforms. Experience working with electronic referral, admissions, CRM, or healthcare management systems preferred. Excellent verbal and written communication skills. Ability to thrive in a high-volume, fast-paced environment. Strong organizational and multitasking abilities. Excellent attention to detail and follow-through. Ability to prioritize competing demands and meet deadlines. Professional, personable, and customer-service-oriented approach. Strong problem-solving and decision-making skills.
