Job Description

MDS Coordinator Meridian Nursing Home, located in Comanche, Oklahoma, is seeking an organized and knowledgeable MDS Coordinator to join our team. The MDS Coordinator plays an important role in ensuring accurate resident assessments, individualized care planning, regulatory compliance, and appropriate reimbursement. The ideal candidate will have strong organizational and communication skills, knowledge of PDPM, and the ability to manage assessment schedules for both skilled nursing patients and long-term care residents.
Responsibilities:
Complete and coordinate comprehensive resident assessments upon admission. Interview residents and gather information from families and the interdisciplinary care team. Schedule and complete MDS assessments according to required timelines, including quarterly, annual, significant-change, and other required assessments. Coordinate the resident assessment and care-planning process with nursing, therapy, social services, dietary, and other departments. Maintain accurate MDS and PPS tracking logs. Monitor skilled residents and ensure documentation supports the services provided. Participate in stand-up meetings, in-services, PPS meetings, triple-check meetings, interdisciplinary team meetings, and other required meetings. Participate in the facility's Quality Assurance and Performance Improvement program. Communicate effectively with residents, families, facility staff, and other departments. Maintain compliance with applicable federal and state requirements and facility policies. Demonstrate regular, reliable, and punctual attendance.
Qualifications:
Current Oklahoma nursing license in good standing. Previous MDS experience is preferred. Working knowledge of MDS requirements, PDPM, care planning, and skilled nursing documentation. Strong organizational, time-management, and communication skills. Ability to manage multiple assessment deadlines accurately. Experience in a skilled nursing or long-term care setting is preferred.
