Role:
As a Permanency Specialist, you'll be at the heart of ensuring that youth receive the care, support, and stability they need to thrive.
You will:
Coordinate Services:
Manage initial and ongoing assessments, ensuring timely placement and appropriate services for children and families.
Facilitate Connections:
Research and recommend relative placements, facilitate family meetings, and help families build robust support networks.
Drive Outcomes:
Develop and implement case plans and permanency goals, support kinship and foster families, and guide children and families through crucial transitions.
Collaborate and Support:
Work closely with court systems, participate in team meetings, and provide guidance to family support workers.
