Currently hiring full-time Care Managers to provide comprehensive care management for individuals with I/DD. The Care Manager will be responsible for providing whole person care management by providing coordination and monitoring that focuses on individual identified needs. The Care Manager will be responsible for leading integrated teams, completing comprehensive assessments, and development of Individual Support Plans. Qualifications:

1. Must qualify as an I/DD QP with a Bachelor's degree in a field related to health, psychology, social work, nursing or other human service area as well as at least two years working with individuals with I/DD. 2. Case management experience (Preferred) Salary is commensurate with education and experience. Benefits are also available.
