Job Description
We Care and Connect Heartland Coordinator General Summary The coordinator, under the guidance of the Advisory Board, will provide continuous support to individuals with disabilities, their families, and caregivers by connecting them with available resources. These resources may include medical, legal, dental services, and more. The coordinator will cultivate a trusting partnership with individuals, families, and caregivers, fostering credibility through knowledge sharing and collaboration to secure and sustain necessary resources. Key responsibilities include effectively managing time, working remotely with minimal supervision, and prioritizing tasks to ensure overall program compliance. The coordinator will maintain an up-to-date database of services while actively seeking additional resources for individuals, families, and caregivers. Additionally, they will oversee the maintenance of the program's website, ensuring all service information is current and making minor updates as needed.
Principal Duties and Responsibilities:
- Suggest resources to individuals, families, and/or caregivers.
- Secure and sustain vital resources for individuals with special needs.
- Maintain strict confidentiality of all regulated information following state and federal laws. Adhere to procedures related to Protected Health Information (PHI) and Personally Identifiable Information (PII).
- Provide culturally sensitive education and guidance tailored to individuals' diverse educational and health literacy needs, using critical thinking to address specific circumstances.
- Effectively manage time, work independently with minimal supervision, and prioritize tasks to meet program objectives.
- Oversee and update the special needs Resources Database.
- Consistently meet quality performance standards and achieve program targets, including monthly productivity goals, to ensure the program's success.
- Demonstrate capability to oversee grant administration and ensure the accurate and timely completion of all required reports. Proficient in managing grant processes, including resource allocation, compliance monitoring, and reporting.
- Proficient in office management skills and working with outside contractors.
KNOWLEDGE AND SKILLS PREFERRED
- Proficient in all Microsoft Office applications and Google Docs
- Proficient in QuickBooks
- Experience in maintaining a website is a plus
- Excellent communication skills (both written and verbal) including public presentation skillset
- Strong organizational and project management skills
- Ability to work effectively in a fast-paced environment with competing priorities.
- Previous experience in customer service or serving customers in some direct capacity.
EXPERIENCE PREFERRED
- 1•2 years of Position Related Experience working in Community Resources • 1•2 years of Position Related Experience working in the field of Social Work, Health Educator, Public Health, and/or related field
Pay:
$23.00 per hour Expected hours: 10.0
