•5:00 PM Position Overview Reporting directly to the Manager of Transitional Services, the Post Transition Service Supervisor oversees program compliance with the Preparation for Adult Living (PAL) contract, Council on Accreditation (COA) standards, and internal Quality Improvement Program guidelines. This position provides direct supervisory leadership to 5-7 case managers and coordinators at the HAY Center, focusing on delivering post-transition services to current and former foster youth as they transition into adulthood.
Key Duties & Responsibilities Staff Supervision & Team Leadership:
Provide direct supervisory leadership to a unit of 5-7 HAY Center case managers and coordinators. Conduct regular individual conferences and unit meetings to review cases, disseminate policy, address operational issues, and perform annual staff evaluations.
PAL Contract & Program Compliance:
Ensure staff achieve HAY Center goals and fulfill PAL contractual obligations with focus on post-transition services. Maintain EVOLV (case management) and PAL (contract reporting) databases, including tracking referral retrieval and eligibility documentation.
Financial & Payment Oversight:
Review case management payment requests for accuracy prior to supervisor submission and monitor youth financial benefits and expenditures.
Reporting & Administration:
Prepare and submit weekly and monthly case management compliance reports required under the PAL contract.
Community Engagement & Event Support:
Participate in collaborative meetings with community partners to stay current on youth resources. Represent the program at HAY Center events as well as statewide and regional PAL activities.
The company is looking for:
Educational Mandate:
Bachelors degree in Education, Psychology, Sociology, Social Work, Criminal Justice, or another Behavioral/Social Science discipline.
Required Work Experience:
Minimum of two (2) years of full-time, paid supervisory experience in a social service environment.
Minimum of three (3) years of full-time, paid experience working in social services with transition-age youth facing challenging life circumstances.
Technical Competencies:
Proficiency using personal computers and office systems, including Microsoft Outlook, Word, Excel, PowerPoint, and dedicated Case Management Software (EVOLV/PAL).
Certifications & Licensing:
Valid Class C Texas Driver's License required upon hire.
Ability to obtain CPR Certification.
Background Requirements:
