Job Description

Regional West Medical Center is seeking a detail-oriented and highly organized Transfer Center Specialist to support patient flow through efficient coordination of interfacility transfers. This role serves as the central communication hub between referring providers, transport teams, and hospital departments, ensuring safe, timely, and seamless patient movement.

A Day in the Life ofYour day starts the moment you log inphones, alerts, and requests are already coming in. As the central hub for patient movement, you quickly begin coordinating incoming and outgoing transfer requests, ensuring every detail is captured and routed efficiently.

Throughout the day, youre in constant communication with transport teams, including ground EMS and air medical services. You track locations, provide updates, and ensure everyone stays informed as patient needs evolve. At the same time, youre working behind the scenes to secure and communicate bed assignments, making sure the receiving team is prepared when the patient arrives.

Between calls, you carefully document each transfer interaction in the EHR and transfer systems, maintaining accurate, real-time records that support continuity of care and regulatory compliance.

In critical moments, especially with air transport, you may also review and communicate weather conditions, helping ensure safe travel for both patients and transport crews.

By the end of your shift, youve handled dozens of moving pieceseach call, each coordination, each decision contributing to timely care for patients who need it most. And while you may not always see the patients face-to-face, your impact is felt in every safe, successful transfer.
