Job Description

Job Summary:
Provides transportation to and from healthcare facilities for residents of Skilled Nursing Facility. This Job also will require some maintenance to the facility at times.
Duties:
Works with management to provide transportation to residents based on transportation schedule. Provides, completes, and documents resident care as assigned in a timely and accurate manner. Attends and participates in facility in-service programs about transportation needs. Reports any change of condition to charge nurse. Must be capable of independently working on the following for transportation Transporting residents to and from appointments Moving residents about the facility Moving residents in an out of hospitals, and healthcare facilities Managing transportation schedule Communicating key information to facility team regarding transportation Transportation position requires a valid drivers license Employee must be able to push, pull, move residents to and from wheel chair.
