Job Description
Summary This position is responsible for performing duties related to patient transitional care management (TCM). In-office presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data in compliance with HIPAA. Minimum Qualifications Minimum completion from an accredited program for medical assistants i.e., Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA.), and Responsible for maintaining continuing education credits and renewal of certification prior to expiration. CPR and First Aid Certified Passage of a criminal background check. Understand and practice the ethics of confidentiality, requirements of Health Insurance Portability and Accountability Act (HIPAA). Valid driver's license and reliable transportation Knowledge, Skills, and Abilities Computer skills using Microsoft (MS) Excel and SharePoint, MS Word, MS Outlook, MS Teams, Acumen, and various data management platforms used by partners of Eastern Nephrology Associates. Ability to function well while involved in multiple task assignments. Ability to understand and follow oral and writer instructions. Ability to meet set deadlines. Possess a willingness to accept orders and perform repetitive tasks. Skill in organizing resources and establishing priorities. Excellent verbal and written communication skills required.
