Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee. Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis. Tuesday- Saturday 10AM-6:30pm Assist MHC staff with transportation for their consumers to essential appointments concerning their treatment and recovery. Must have F Endorsement.
Position Responsibilities:
1. Transport consumers to and from designated appointments. 2. Transport medications to be delivered to consumers. 3. Communicate with the team regarding consumer concerns and/or transportation issues for the consumer. 4. Attend team, department and Agency meetings as required. 5. Maintain the MHC vehicles in optimal condition and report any concerns to the Transportation Supervisor. 6. Must inspect assigned vehicle at the beginning of work shift and complete vehicle inspection form. 7. Complete all required paperwork in a legible and timely manner. 8. Respond expediently to all communication with office. 9. Responsible for managing route, which includes calling consumers prior to leaving the office and planning route effectively.
Other Responsibilities:
Responsible for attending all mandatory in-services, continuing education and annual health requirements Comply with all Corporate Compliance policies, practices and procedures Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator Will be compliant with the relevant regulations concerning the privacy and security of consumer's protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Participate in proactive team efforts to achieve departmental and company goals Perform other duties as assigned Provide leadership to others through example and sharing of knowledge/skill • Follow all policies and procedures set by the Agency
