COMPANY SUMMARY
Well&You is a fast-growing multispecialty healthcare practice and medical brand building a better way to care across Northeast Florida. Across three state-of-the-art clinics, office-based labs, and ambulatory surgical centers, our board-certified physicians and advanced practice providers deliver coordinated chronic disease management, cardiovascular and vascular care, limb-salvage and wound care, cardiac monitoring, women's health, non-opioid pain and spine care, and advanced outpatient imaging and procedures. With more than 20,000 square feet of clinical space and a growing network of providers and locations, we combine the capabilities of a sophisticated specialty platform with the speed, ownership, and opportunity of an entrepreneurial organization.
ABOUT WELL&YOU RURALREACH
Well&You RuralReach will open four permanent satellite clinics across Northeast Florida, including rural Baker and Nassau counties, bringing chronic disease management, vascular and limb-salvage care, cardiac monitoring, women's health, and non-opioid pain care to residents who now drive 35 to 40 miles for specialty care. In communities where heart disease, diabetes, stroke, and amputation rates run well above Florida's average, this award turns Well&You's long-standing local roots and relationships into permanent access points for care. Staffed by board-certified physicians and APRNs, these clinics will build sustainable rural health infrastructure and bring a better way to care to rural Northeast Florida.
WHY THIS ROLE MATTERS
This is not a routine site-administration job. The RuralReach Administrator will be the operational builder behind a new four-clinic network: turning an award and a strategic plan into sites that open on time, earn community trust, deliver reliable access, and operate sustainably. The role offers broad ownership, direct visibility with leadership, and the chance to shape the operating model from the ground up. As RuralReach expands, the scope can grow into leading a multi-site rural operations team and contributing to broader Well&You growth initiatives.
ROLE SUMMARY
The RuralReach Administrator owns the launch, day-to-day performance, and continuous improvement of Well&You's rural program from the operational side. You will coordinate sites, people, schedules, workflows, community and referral relationships, budgets, vendors, data, and grant compliance across the network. You will move comfortably between strategy and execution - building the plan, solving what is blocking it, documenting the repeatable process, and using data to improve access, quality, productivity, and financial sustainability. The role is AI-forward and highly cross-functional. You will use eClinicalWorks, spreadsheets, dashboards, and AI-assisted tools to automate recurring work, create a trusted source of truth, and give leaders timely recommendations rather than raw information. The strongest candidate is a decisive healthcare operator who brings solutions rather than escalations and can lead effectively even when a supervisor is not in the room.
INITIAL MANDATE
Bring four permanent RuralReach satellite clinics from plan through launch and stable operations. Create one repeatable operating playbook for site readiness, staffing, scheduling, patient flow, referrals, outreach, compliance, reporting, and issue escalation. Build the local partnerships and referral engine needed to make each site trusted, productive, and responsive to community needs. Establish grant-compliant budgets, documentation, metrics, and reporting that stand up to leadership, funder, and auditor review.
KEY RESPONSIBILITIES
Launch and scale the network. Lead the operational opening and stabilization of four permanent rural clinics, including site selection and lease coordination, buildout and outfitting, licensure and accreditation readiness, equipment, IT and telehealth connectivity, workflow design, and go-live. Own multi-site operations. Run day-to-day rural clinic operations and outreach events, including readiness, opening and closing procedures, staffing coverage, supplies, equipment, patient flow, safety, and rapid resolution of site issues. Build and lead the team. Coordinate recruiting, onboarding, training, scheduling, and performance support for rural staff; partner with clinical leaders to ensure each clinic day is appropriately staffed, clinically supported, and productive. Create reliable access to the full care model. Coordinate with physicians, APRNs, and clinical teams so patients can access chronic disease management, vascular and limb-salvage care, cardiac monitoring, women's health, and non-opioid pain care, with timely follow-up and clinically appropriate connection to hub-based specialty services. Develop community and referral partnerships. Build durable relationships with rural primary care practices, rural health clinics, FQHCs, critical access hospitals, EMS, county health departments, senior centers, employers, and community organizations. Turn outreach into ongoing care. Plan and execute community education, screening, and health-literacy events; own logistics, staffing, materials, patient intake, follow-up, and conversion of identified needs into scheduled care. Own grant and regulatory execution. Maintain audit-ready documentation, monitor allowable costs and time-and-effort requirements, prepare grant deliverables and progress reports, and coordinate compliance with HIPAA, OSHA, infection control, telehealth, licensure, and documentation standards. Build the operating dashboard. Define, validate, and report the metrics that matter - patients served by county, new patient volume, wait times, no-shows, schedule utilization, referral conversion, service-line use, screening yield, follow-up completion, and documented access barriers. Manage the economics. Track budget performance, volume, payer mix, cost per site day, and contribution; flag variances early and recommend changes to staffing, site cadence, workflow, or service mix that improve sustainability without disrupting hub operations. Own vendors and enabling infrastructure. Manage contracts and performance for space, transport, equipment, supplies, imaging, connectivity, and other rural operations partners; keep renewal dates, costs, responsibilities, and dependencies visible. Make improvement repeatable. Find the root cause of recurring failures, redesign the process, document it in clear SOPs and checklists, train the team, and audit whether the fix is working across every site. Represent Well&You locally. Serve as a credible, responsive on-the-ground representative with referral partners, patients, community groups, vendors, and local stakeholders; travel regularly to rural sites and outreach events to lead firsthand.
HOW SUCCESS WILL BE MEASURED
Four clinics launched and stabilized against approved timelines, budgets, and readiness standards. Reliable clinic-day coverage, site readiness, schedule utilization, and patient follow-up. Growth in patients served, referral conversion, access by county, and use of needed service lines. Timely, accurate, audit-ready grant reporting with no missed deliverables or unsupported costs. Improving site economics, controlled budget variance, and evidence-based recommendations to leadership. Documented, repeatable workflows that reduce operational failures and allow the program to scale.
REQUIRED SKILLS AND ABILITIES
Builder's mindset: proven ability to take a site, service line, or program from concept to go-live and then operate it with discipline.
High ownership and sound judgment:
identifies the problem, owns the next step, and arrives with a safe, compliant, cost-effective recommendation. Strong foundation in outpatient practice operations, staffing, scheduling, patient flow, revenue-cycle basics, credentialing, payer enrollment, and healthcare compliance. Program and project management skills sufficient to run multiple site launches, workstreams, and event calendars without dropped commitments.
Data discipline and financial fluency:
can define a metric, validate the data, read a budget, explain variances, and translate findings into action and dollars. Confident, concise communication with executives, providers, staff, funders, referral partners, community organizations, vendors, and local officials. Technology fluency, including eClinicalWorks or a comparable EHR, advanced spreadsheets, dashboards, and practical use of AI-assisted workflows with appropriate verification. Cultural competence and ease working in rural communities, including with patients and partners facing transportation barriers, limited health literacy, or long-standing distrust of the healthcare system. Valid Florida driver's license, reliable transportation, and willingness to travel regularly to rural sites and outreach events.
EDUCATION AND EXPERIENCE
Bachelor's degree required; substantial relevant healthcare operations experience may be considered in lieu of degree. Master's degree in health administration, public health, or business preferred. At least five years of healthcare operations, practice management, or program administration experience with direct accountability for outcomes. Experience opening a new healthcare site, service line, or program from the ground up strongly preferred. Experience administering grant-funded programs, including budget tracking, time-and-effort documentation, and funder reporting, strongly preferred. Experience in rural health, community health, an FQHC, a rural health clinic, or a critical access setting strongly preferred. Working familiarity with credentialing, payer enrollment, and multi-location place-of-service requirements.
WHAT MAKES SOMEONE SUCCESSFUL HERE
Accountable:
Takes ownership of decisions, quality, and deadlines.
Ambitious:
Wants the organization, the team, and their own scope to grow.
Calm and unflappable:
Prioritizes clearly and keeps moving when several demands compete at once.
Intellectually curious:
Learns new clinical, operational, financial, and regulatory topics quickly.
Resourceful:
Finds workable answers in environments where the ideal resource may not be close at hand.
Strategic and detail-oriented:
Keeps the larger goal in view while protecting execution quality.
Clear communicator:
Shares decisions, risks, and next steps concisely across all levels.
High integrity:
Protects patients, data, public funds, and the trust of rural communities.
Team-first leader:

PHYSICAL AND TRAVEL REQUIREMENTS
Prolonged periods of sitting at a desk and working on a computer. Regular travel between the Jacksonville area and rural clinic sites, including some early starts and occasional evening or weekend outreach events. Ability to lift up to 25 pounds and to load, transport, and set up equipment, supplies, and event materials for offsite clinics and community events. Ability to work productively in smaller rural facilities, construction or buildout environments, and temporary event spaces with limited support staff on site.
