We're seeking an experienced Home Health Administrator / Director of Operations who is passionate about clinical excellence, operational performance, and team development. This is a senior leadership opportunity for a licensed Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) with proven experience in home health operations, Medicare compliance, and census growth . Position Summary The Administrator / Director of Operations - Home Health provides strategic and operational leadership for the home health agency. This role is responsible for clinical oversight, regulatory compliance, financial performance, staff leadership, and patient outcomes , while driving growth and maintaining the highest standards of care. Key Responsibilities
- Provide overall operational and clinical leadership for home health agency operations
- Ensure compliance with Medicare Conditions of Participation, state regulations, and company policies
- Oversee OASIS accuracy, PDGM and RCD compliance, and QAPI initiatives
- Drive census growth through referral management and market engagement
- Ensure high‑quality, patient‑centered care and interdisciplinary coordination
- Lead budgeting, financial performance, and productivity management
- Recruit, develop, and retain high‑performing clinical and operational staff
- Maintain effective infection control, patient rights, and risk management programs
- Utilize MatrixCare and performance data to drive outcomes
