About Swan Primary Care Swan Primary Care is a rapidly growing physician-led healthcare organization providing care across multiple settings throughout the Chicagoland area, including: Primary Care Clinic Hospital Medicine Skilled Nursing Facilities (SNFs) Assisted Living Facilities Independent Living Communities Post-Acute Care Transitional Care Management Value-Based Care Program We are seeking a highly organized, driven, and accountable Practice Manager who can oversee daily operations across the organization and ensure our clinical, administrative, and operational teams work together efficiently. This is a leadership position reporting directly to the physician owners. Position Summary The Practice Manager serves as the operational leader of Swan Primary Care. Rather than directly performing billing, accounting, marketing, or HR duties, this individual oversees those departments, coordinates projects, solves operational problems, tracks performance, and ensures every part of the organization runs efficiently. This role requires someone who thrives in a fast-paced environment, enjoys building systems, leading teams, and improving workflows. Responsibilities Operations Leadership Oversee day-to-day operations across all service lines Ensure smooth coordination between outpatient clinics, hospitals, SNFs, assisted living, and independent living operations Develop and improve operational workflows Lead operational meetings and ensure accountability Monitor key performance indicators (KPIs) Team Management Supervise and coordinate : Billing Department Accounting HR Marketing Front Office Medical Assistants Virtual Assistants Operations Coordinators Clinical Leadership Ensure departments communicate effectively and projects are completed on time. Project Management Lead initiatives such as : Opening new practice locations Launching new clinical services Provider onboarding Facility onboarding EMR implementations Operational process improvements Vendor management Technology implementation Physician Support Remove administrative burdens from physicians Resolve operational issues before they impact patient care Coordinate provider schedules and staffing needs Improve communication between clinical and administrative teams Financial & Performance Oversight Work alongside the billing manager and accountant to monitor: Revenue trends Productivity Collections Staffing efficiency Budget performance Operational expenses Quality & Compliance Maintain regulatory compliance Ensure policies and procedures are followed Monitor operational quality metrics Coordinate credentialing and provider onboarding Oversee facility contracts and operational documentation Ideal Candidate We're looking for someone who : Thinks like an owner Is highly organized Can manage multiple priorities simultaneously Is comfortable leading teams Solves problems independently Communicates exceptionally well Uses data to make decisions Holds others accountable Continuously improves systems and processes
Qualifications Required :
Bachelor's degree 5+ years of healthcare management experience Experience managing multidisciplinary teams Strong leadership skills Excellent organizational and communication skills Experience with healthcare operations Proficiency with
Microsoft Office and Google Workspace Preferred :

$70,000.00 - $95,000.00 per year
