Location Fairbanks, AK, US Organization Name Health Services Job Summary Job Summary:
Job incumbent will provide coordination of client care for the Home Care Services Program in the Department of Health Services. The Care Coordinator is a healthcare professional responsible for scheduling, organizing, and managing all aspects of healthcare maintenance and treatment for their patients Duties include helping patients complete paperwork, communicate with healthcare teams about patient's treatment plan, and educate patients about resources and options for managing their health. Adhere to the TCC Ch'eghwtsen' model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.

Representative Duties:
Under general supervision of the Home Care Manager job incumbent will: 1. Provide patient care coordination services as defined by the State of Alaska for Home Care and Community Based Medicaid Waiver applicants. 2. Perform assessments and document the patients care needs including referrals, support systems, plans of care and specific medical or physical needs. 3. Develop and document patient centered written care plans. Review and revise the plans of care on a regular basis for Home Care Providers to carry out. 4. Works directly with the State of Alaska to process Medicaid Waiver applications and plans of care. 5. Provide continuing education units to Home Care Providers including but not limited to: CPR, first aid, and continuing education units. 6. Coordinate with Galena and Tanana assisted living facility and assist with staff training. 7. Communicate with Home Care Providers, clients, family, Health Aides, health professionals, Tribal Councils, and others regarding client health care needs. 8. Assist with implementation of program activities as directed. Assist with program needs analysis. 9. Attend and participate in meetings and online based teleconferences as required. 10. Must maintain strict confidentiality in accordance with HIPAA standards. 11. Travel to remote villages may be required for this position.

Minimum Qualifications Minimum Qualifications:
1. Diploma in Nursing. Bachelors of Science in nursing (BSN) preferred. Active state of Alaska license as registered nurse. 2. Five years experience working as a Registered Nurse. 3. Certification of Care Coordination from the state of Alaska or obtain certification within three (3) months of employment. 4. Two (2) years' experience in Home Care long term care of delivery of community based services. 5. Previous experience working with Alaska Natives or other cross-cultural experience. 6. Must pass background check pursuant to Alaska DHSS, federal Indian Child Protection and Family Violence Prevention Act requirements. 7. Previous work experience working with the elderly.
Knowledge, Skills and Abilities:
1. Excellent communication skills to effectively coordinate care between family, legal representatives, client and the state of Alaska. 2. Ability to manage multiple projects, prioritize tasks, and ability to work independently. Effectively manage time and meet role expectations is essential. 3. Communicates plan of care, including changes and issues related to plan of care to client/family, physicians and other members of the healthcare team. Facilitates communication between the physician, interdisciplinary team, patient, family and the state of Alaska. 4. Must be able to work with electronic medical record, Microsoft office and State of Alaska state reporting systems. 5. Maintain confidentiality and complete annual HIPAA training.
Supervision:
This position has no supervisory responsibilities.
Supervision Physical demands:
Frequent travel to interior villages required. Incumbent may be expected to work extended hours. Conditions include but are not limited to: travel by small un-pressurized aircraft, poor heat, lack of running water, and adverse weather conditions. Position requires bending, stooping, kneeling, reaching, stretching and a considerable amount of walking. Shared office space. Job incumbent may be required at times to lift up to thirty pounds in support of equipment and supplies. Incumbent must be able to work accurately in a busy and sometimes stressful environment. Job incumbent may travel to remote locations by small plane or boat to assist with care coordination.
