Our Office Coordinator Will:
Provide exceptional and timely support to patients, families, visitors, and community partners. Develop meaningful, effective, and professional relationships with communities, families, and patients. Foster a culture in alignment with the Heritage Core Values. Support other OnCare Health associates as needed to ensure a positive, team-focused work environment. Engage in process improvement initiatives in customer relations, quality assurance, financial management, and staff development as measured by QAPI program development and survey outcomes (CMS, ACHC, or CHAP). Answer incoming phone calls and greet visitors and vendors in a professional and courteous manner, and respond to needs and requests promptly. Assist with intake and referral processes including data entry, requesting medical records, and scheduling admissions. Assist with the maintenance of electronic patient records and ensure all uploaded documentation is accounted for upon admission and throughout patient length of stay. Complete routine chart audits to ensure regulatory compliance. Assist with human resource processes, maintaining employee files, hiring, and onboarding. Assist with process and performance improvement projects. Monitor communication channels for the OnCare Health team and provide staff assistance with DME orders, supply requests, and patient electronic charts. Assist in the management of patient, office, and kitchen supply inventory.Our Office Coordinator Has:
A high school diploma or equivalent. Excellent verbal and written communication skills with the ability to navigate challenging and ambiguous situations involving emotions or conflict. Excellent organizational and time management skills. Strong technical skills with knowledge of business software programs including Word, Excel, SharePoint, Outlook, Teams, and PowerPoint. The ability to handle unexpected challenges and problem-solve effectively. A willingness to adapt to changing tasks and priorities. Attention to detail. Previous hospice administrative experience (preferred). A valid driver's license and acceptable driving record. Why Join Us as anOffice Coordinator:
