Description The Clinical Program Coordinator is an entry- to intermediate-level opportunity for those with basic office skills to gain experience in clinical care coordination and outreach. Roots Community Health is seeking a Program Coordinator for The Cut Hypertension Program (CHP) in Sacramento. The Cut Hypertension Program (CHP) is an initiative started in 2010 borne out of evidence that African American barbershops/salons can be places of health prevention, community building, outreach, and treatment for African American men, a community affected by alarming health disparities in cardiovascular disease.
Duties and Responsibilities:
Identify and engage barbershop/salons and cosmetology businesses in the Sacramento area to join the Cut Hypertension Program network. Discuss programmatic expectations with partners and explore how services can be effectively delivered in their spaces. Oversees ongoing barber health coach training and manages partnerships with barbers and barbershops/salons, including communication, documentation, soliciting any needs, dropping special payments, incentives, and trained barber roster management. Identify key primary care and health system stakeholders. Promote a comprehensive referral and data-sharing system with primary care providers to support collaborative care for patients with uncontrolled hypertension at partnered barbershop/salons. Oversee the recruitment, training, scheduling, and coordination of volunteers for blood pressure screening events at partnered barbershops/salons. Oversee the review of positive blood pressure screenings or outstanding community clinic referrals for targeted scheduling. Provide support scheduling patient appointments, monitoring and tracking attendance, and performing other specialist duties which may include collecting initial demographic information, verifying insurance eligibility for all program participants, and creating and managing their medical charts. Oversee a panel of program participants under the clinical provider's care and manage corresponding data collection. Use this data to inform the medical director of any strategic interventions needed to enhance the quality and impact of the program. Develop processes to support program participants in navigation to establish or reconnect with their primary care home to maintain successful care coordination and collaboration on chronic disease management throughout the relationship with the program. Ensure that each barbershop/salon is equipped with the necessary marketing materials and supplies to effectively deliver program services. Create marketing, outreach, and training materials across an array of mediums (print, web, and mobile). Design and conduct activities, such as surveys and focus groups, to assess program performance and guide strategic planning.
Requirements:
Requirements Attending weekly virtual team meetings and in-person meetings Demonstrated experience in community engagement or community health work Demonstrate excellent written and verbal communication skills, along with proficiency in Excel and the ability to learn and use survey tools effectively. Ability to work on and oversee multiple projects, ensuring timely and effective completion. Interest/proficiency in working on issues affecting urban underserved, impoverished and marginalized communities of color
